Media Permissions Request Form
Abstract
This form can be used alongside the OER permissions form, and is to gain consent to use print / audio or visual content belonging to others. For example if you create a video or recording, you will need to gain permissions from the individual to use it for the manner which you are specifying.
These forms are based on the Jisc versions, and you can download the Word document and insert your own logo and details.
